Patient-centered care
Meeting with physicians and other medical experts usually requires scheduling multiple appointments across several days or weeks and spending time traveling to various locations. At City of Hope, we want you to get started healing as quickly as possible. Our patient-centered care model puts you at the center of your treatment, involving you and your caregiver throughout the decision-making process.
Each patient has a team of multidisciplinary cancer experts who work together to design and deliver a cancer care plan tailored to your individual needs. All the members of your care team, which may include a medical oncologist, surgical oncologist and radiation oncologist, are experienced in treating cancer with state-of-the-art tools and technologies.
Depending on your needs and preferences, your care team may also include supportive care clinicians—a pain management doctor, registered dietitian, oncology rehabilitation provider or other care providers—to help you manage side effects.
How does this approach improve your cancer care?
Your care team works under one roof, and collaborates regularly with you and one another, monitoring your progress and adjusting your care plan as necessary.
Having a dedicated team provides many benefits, including:
- Speed of care: With a cancer diagnosis, it is critical to get an appropriate treatment plan as quickly as possible. Our patient-centered approach is built for efficiency and convenience, with an emphasis on eliminating waste in time and resources, so you get results more quickly.
- Individualized treatment: When the members of your care team are chosen based on your needs, cancer type and treatment goals, you have the peace of mind of knowing that your care plan is tailored to you.
- Better communication among your doctors: Your care team communicates in real time and makes adjustments or orders additional tests as needed during your initial evaluation and throughout your treatment.
- Patient empowerment: You and your caregivers are involved throughout the treatment planning and implementation process, allowing you to make informed decisions about your care.
- Coordinated care: Your care team works together, all under one roof, coordinated by our care management team. We orchestrate your schedule, communication, education and follow up so you can focus on healing.
